Faith-based K–12 schools in Southeastern Kentucky may apply to become a partner school. Submit this form to begin the partnership process and help eligible students access need-based scholarship assistance.
Verify student enrollment and attendance.
Verify tuition, fees, and other tuition assistance.
Notify SEKY if a scholarship recipient withdraws, transfers, or changes enrollment.
Apply scholarship funds only to eligible educational expenses.
Maintain records supporting scholarship payments.
Cooperate with reasonable requests for documentation.
The undersigned certifies that the school agrees to the responsibilities described above.
